Approach the office concerned revenue authority i.e, Tahsildar and submit necessary application in the prescribed format for mutation of land in the name of mother enclosing the Death Certificate of father and also no objection from other legal heirs to have Phani & RTC mutated in the name of mother.

The property stood in the Fathers name and as per law of succession each child becomes joint owner having 20% undivided right and share in the land.If you want to transfer the property in your mothers name, all the children have to release their respective share through a registered released deed in favour of your Mother.
