How to claim ownership in father's property

I m alone to my father my father is suffering from paralysis he understands everyting and can speek only little things like yes or no, he cant walk properly,so what procedure to do for legal ownership of my father one is my house and another is a shop which is on loan,want should i have to for legal ownership after my father i want to give 50% shares to my mother.
