What is the procedure to file a contempt of court application in CAT

hi, This is related to a Service matter, in which Central administrative Tribunal granted a stay order on disciplinary proceedings.But a stay order was not honoured by Railway officers. Therefore I am about to file a contempt petition in Central administrative Tribunal, Mumbai. I know the format of application, but I want to know whether the application should be accompanied by an affidavit separately on stamp paper, or whether the affidavit can be part of contempt of court application.
The petition shall be supported by an affidavit verifying the facts relied upon except when the motion is by the Attorney General or the Solicitor General or the Additional Solicitor General;
