Is disclosing family medical history necessary for applying for job

Sir/Ma'am In an medical exam(civil service) they have a statement asking for my parent health status-if alive,age and health status or if dead,age and Cause of death - and in my recent finding my father have died due to retro viral illness(hiv) and mother too is affected while performing her duty as wife, but I'm negative and fit.DO i need to disclose this about parent health status and if not Am I legally Bound as at the end of the form it states "any wrong information may lead to termination of service in future", please help me out Sir/Ma'am please

You can state the cause of death by stating them to keep it confidentially and don't go to disclose it with your college or hostel mates if you are not comfortable. You don't have to feel guilty for anything which is your beyond your control and you have not done any mistake in this, hence your life is precious and don't waste it by sticking on to thinking some thing useless and just learn to move on.
