Affidavit Documents required for legal advice in india

Hi
I'm a prospective nri student who has applied to management colleges in maharashtra in the nri quota. I was asked to submit the following documents:
1) "Affidavit of Family Chart Duly Signed by Sponsor Making Clear Relationship" and
2) "Affidavit of Candidate / Sponsor disclosing his / her Full Identity i.e. Full Name, Age, Residence, Occupation, Relationship with Candidate Duly Signed by the Candidate / Sponsor."
The word sponsor refers to an NRI guardian(here, my father who is an NRI).
I would like to know if these documents are available at the court offices or are these just self-made documents to be signed by my father?
Do they need to also be signed/stamped by a legal authority since they are affidavits?

This being affidavit has to be solemnly affirm before a authority who is authorised to administer oath now that this document being based upon the requirement and facts have to be drafted accordingly
Second aspect being government of India have declared that certain affidavit need not be notarized hence this document unless insisted be signed and submitted but to be on safer side notarized it.

All the aforementioned documents need to be signed by you if it's your affidavit or your father's if it's his affidavit also that these affidavit needs to be either affirmed by court registrar or be notrised by public notary . These have to be made by you.

All sorts of Affidavits has to be on stamp paper. Make all those Affidavits on a hundred rupees stamp paper you can write the affidavit in your own words as per the requirements and then notarize the same. All sorts of Affidavits has to be notarize.
