What is the procedure to change the name and date of birth in passport

I have a passport which expiry is above 3year.i have an issue with my passport.my dad made my passport at that time he was not having my dob so he created a new one by an agent and by that details my passport was created .there are many errors in my passport like dob,place of birth,name.i have a dob certificate stating that I was born in my home which has correct details which I want in my passport.i want to change my name like I want to add Mohd before my name in my passport bcoz every certificate have Mohd before my name like ssc ,aadhar card,dob certificate.dob it 9th month I want it as 8th
Sir,
1.In the instance, file a "CIVL SUIT" (Declaratory Order) in the Civil Court with in your Jurisdiction, of your residence, it can be filed on the basis of "Documentary Evidence" , Educational Certificate/Election Identy Card, which is apparent of date of Birth.
2. Subsequently the court/Magistrate will order for General Public Notice in newspaper, and he will register your statement on oath and on the same day he will pass Order for Change of Birth Date as per your documentary evidence.
3.Change of name: fill the new form with rectified particulars and submit the same to the Regional Passport Officer along with your old passport , affidavit and the evidence of the correct name.
