What is the procedure for changing the name after marriage in docs

My name is surbhi Kumari and i recently got married now i want to change my name and add my husband's name and title wid my name i.e. surbhi shashank dwivedi. I want to change/ update all my legal documents like pan, aadhar voter with that name. What procedure do i have to follow for that. And how much time it will take.

In this case you need to make an affidavit.
