what can be done in respect to different names on documents

I am applying for government jobs. I always face a problem when entering the name where specified as "name as per 10th marksheet". My name differs from 10th marksheet as per Maharashtra state format....i have an affidavit supporting these names.....so my question is what name should I append...as per 10th OR as per affidavit.
The right procedure is getting a notice published in a local News Paper, regarding your intention to change your name/or add surname to it, and subsequently get it published in Gazette of India.
These days Gazette prints have been discontinued, so you may have to check with concerned press in your city, if they still print the Gazette Notification, or it is given to you in soft copy.
so the name changed would be the correct name and you have to use that name.
