Spouse name mentioned by mistake in passport how to change it

Hello sir / madam In my old passport spouse name is different because I engaged a girl and passport agent puted her name as spouse by mistake on 2002, it's my mistake reshan card also make with her and we separated with our family settlement. Then I got married another girl and all legal documents like marriage certificate, aadhar card etc., I have applied a passport application for spouse name change on January 2016 so they sent a notice to submit divorce decree, but without marriage how to possible to get decree ? That girl also married and she will never support. Give me right suggestion.
if you were never married then how can the agent write the name of the girl as spouse as you require document to be submitted. Secondly, was the settlement between the families a written agreement or verbal. you need to write to the passport clearing the things. if and so you were married you need to get the divorce decree until and unless you submit it the name will not change
