Retain married name on passport after divorce

Hi, I am divorced and need to renew my passport, but don't plan to change my name ack to my maiden name. Can you advise whether I need any special documentation when I apply for passport renewal? Their web site asks for Annexure D, but I don't plan to change my name. Please advise.
every Indian woman is free to use any name it could be maiden , name if changed after marriage or any other name by choice adopted.
In this case you have made choice to maintain married name after divorce. perfectly fine. only disclose your marital status as divorcee or unless you are re-married to someone.
