Birth certificate name change is an offline process and there are very limited states which have started porting birth certificate related content to online clouds. Therefore, the procedure to change name differs from state to state

If you are following offline procedure, then you need to visit nearest birth certificate office and ask for the concerned documents/forms. Once you have the birth certificate name change form, fill it completely, attach all the supported documents and submit it to the concerned officer.

Also, you will have to make a name change affidavit stating the reasons for name change and then you have to advertise in two leading news papers and give gazette notification, if required. Court order may also be necessary in certain cases.

The authorities, who issued the birth certificate, will verify your details and if they are convinced with your name change, your name will be changed in the birth certificate and you will be issued a new birth certificate carrying your changed name. In normal cases it takes around 7-10 working days to get name change in birth card.

Documents Required for Birth Certificate Name Change
You will require very basic documents to support your name change which are as follows:
§ Newspaper cutting where you have given the name change advertisement
§ Written proof from local level bodies like Gram Panchayat or Municipality Office

While submitting the name change form, you will have to attach any of the above proof on basis of which the invigilator will validate and change your name in the existing certificate. Once your name is changed in the birth certificate, you can use it as base to get name changed in other documents like your mark sheets, ration card, passport, PAN card etc.
