Procedure of name change of a child

I have a child of 1.5 years, I want to change his name. Could you please tell me the exact procedure?
First u have to make affidavit mentioning old details of child along with new name that u r looking from change. secondly u required publication in news papers in English and regional language and thirdly published in gazette notification. If u need help revert to.

Well, an affidavit in his/her name by both the parents.
Public Notice to be published.
Then the name to be published in official Gazette.
And, thereafter basis official Gazette, an application to Registrar of Birth and Death for issuance of fresh birth certificate.

As the your child is only one and a half years old, thus, you will have to apply for the change only in birth certificate, procedure for the same is as follows:
Go to the birth registration office and get the application form which is the same as a fresh application, and submit it with the following documents:
Hospital letter
Other ID proofs
School certificate photocopies should have the same name as in the id proof
Affidavit stating that the name is incorrect and you seek to change or make a correction in the existing birth certificate.
After submitting the document, they may give you a date to go and collect the birth certificate.
