Procedure for Name Change

Sir I am a resident of Uttar Pradesh residing in Delhi. I want to change my name, as it is not feasible for me to go home and find a notary Lawyer my question is
"CAN ANY NOTARY LAWYER ISSUE NAME CHANGE AFFIDAVIT EXCEPT YOUR LOCALITY"
AND SECOND IS IT NECESSARY TO PUBLISH MY NAME UTTAR PRADESH GAZETTE OR IN INDIAN GOVT GAZETTE PUBLICATION WILL BE SUFFICIENT.
