Procedure for Change of parent name in birth certificate

My mother and father's names are wrong in my birth certificate. I want to apply for a fresh passport. Can you tell how to go about the process. The residential address is also of the time when i was born, would i have to get that too changed?

There is simple procedure for the same. You Will need to submit the following documents to the Registrar of Births & Death in your Municipality along with an application-
A. Marriage certificate of your parents with their correct name mentioned on it,
B. Hospital documents, such as discharge papers where the name of your parents and your date of birth is clearly mentioned.
C. An affidavit made on a Rs.100 Stamp Paper, duly attested by a Class I Magistrate
D. Copies of 'change of name' advertisement published in two local newspapers
You can also have the change of name published in the Government Press in your State. The same will then be published in the gazette, copies of which you will need to attach with your application.

If name of your parents are incorrect in your birth certificate you can oath on Affidavit and further documents can be corrected if same mistake is done on them ...i need complete info to answer you properly ....!!
