How to include my name in birth certificate

Hi, My birth certificate doesn't contain my name on it and I am finding difficulty to get my passport done. My date of birth is 12-12-1989, please advice me how to include my name in my birth certificate. Please advice me the procedure Thank you

The BBMP office is the authority that provides Birth Certificates. You can apply for a birth certificate by downloading a form from the BBMP website ( http://218.248.45.169/download/welfare/birth.pdf ) and submitting it to the BBMP.

(1)Get a letter from your school attesting your photograph and name. Submit the same along with the requisition letter to your local municipal authorities. The said authorities will issue birth certificate along with your name.

You can prepare an Affidavit stating that due to omission, inadvertance and oversight your name was not included in the birth certificate. This sworn affidavit has to be notarized by a Notary Public, thereafter you go to the concerned authority in Bangalore it is BBMP request for an application for name inclusion and state your name in the application. It should be done.
