To get your name changed in the Birth Certificate you should contact to a Registrar and a Registrar has an authority delegated by the Government where the Government may appoint a Registrar for each local area comprising the area within the jurisdiction of a municipality, Panchayat or other local authority or any other area or a combination of any two or more of them.
For the purpose of changing the name as per section 15, of THE REGISTRATION OF BIRTHS AND DEATHS ACT, 1969
(Act No. 18 of 1969), you can apply to the Registrar with an affidavit. and the name will be changed pertaining to the satisfaction of Registrar by giving a reasonable excuse for doing the same.

Hello,

This is the basic work which will be carried on by your local body. You will have to submit an affidavit before the authorities with other documents to show that you have changed your name and is not the same as is appearing right now in the birth certificate. The procedure usually takes 1-2 months.
