Different name on different documents how can I apply for passport

In SSC and HSC certificates my name is printed MADHAV KUMAR PATEL. And in PAN, Bank Passbook, Aadhar, Voter Id, College certificates it is MADHAV PATEL. I want to apply for passport. What step should I take. Do i need to make affidavit.
You need to make a formal application to every concerned department for the correction in the spelling of your mother’s name with a proof of the correct spelling such as birth certificate.
