Different name in different documents how to change it

My mothers actual name is Panchi and her name in my birth certificate and her all documents is Panchi but in my all other documents it is Prachi .Now i wish to apply for passport so what can be the solution for this problem as i will have to provide my birth certificate along with other documents.can any affidavit serve this problem please let me know.
You need to make a formal application to every concerned department for the correction of the spelling of your mother’s name with a proof of the correct spelling.
