Applied for passport 3years back can I apply again from different city

I have applied for a passport in Kolkata before 3 years. Six Months before I enquired about my passport status they said it was closed due to I was not present in the given address at the time of verification. Now I am staying in Mumbai and want to apply for a new passport but I forgot the File Number..and there is a column to put previous file Number...What should I do? Can I apply once again without providing previous file number? Will it be a supression of information? Please Help

You can apply for the passport by providing an affidavit stating the facts and reasons as to how you could not be present at the previous residence at the time of verification and go forward.
