To make the Annexure-C affidavit, you will have to follow these guidelines:

Use our pre-drafted affidavit form to fill in your details.
Get it printed on a Stamp paper of minimum denominations like Rs 10 or Rs 20.
Place your signature at the designated place at the bottom of the affidavit.
Get the affidavit attested by First class Judicial Magistrate or Executive Magistrate or Notary Public.

Thus, you can obtain the signature from abovementioned authorities anywhere in India although, it is advisable to take the signature from the jurisdictional Judicial Magistrate or Notary public.
