Just file a suit with the husband old available address u have or use husbands parents house address to serve summons, get a exparte decree, which state do u belong..mere stating the time of sepration is enough, in court and getting a court order is enough to prove the seperation anf u can apply for ration card using the divorce order

You can execute a self declaratory affidavit that you are separated from your husband for the past 4 years.
You can also get a certificate to the extent that you are living away from your husband for the past 4 years from a person of repute like MLA, counselor etc ofyour area you live.
