Police is not registering the FIR

Hi.I have been following up with police for registration of FIR for last 3 months.This regarding my harassments by company who terminated me and during termination illegally confined me and committed other criminal and illegal acts against me.Police sent a letter after 3 months, stating i should take matter to labour court and have disposed off 2 complaints. I am in-person in my labour matter as well as regarding this whole FIR procedure.If someone can give me a sample format for section 156(3) application to magistrate, it will be greatly appreciated.If not, can someone tell me the technical/legal requirements for this application.If I attach my 3 police compplaints and make an aplication to magistrate stating please register these complaints under sec156(3), will that suffice? I consulted with a lawyer and he asked for Rs 50,000 for this application.Better tomake application myself, I cant really afford that kind of payment for one application to the magistrate.
