Yes, the case can be transferred by the concerned High Court if the same is pending within the same State and if the case is pending in for example Punjab and you want to get it transferred to Haryana, then it can only be done by Supreme Court of India. For getting the case transferred, you will have to show the threat perception. Merely alleging threat would not be enough but you need to apprach the concerned Police Station or the judge by filing a complaint to them also.

Threat to life may be considered as a valid ground for transfer of case, however in the event its a criminal/State case, you may just be an witness, with state prosecuting the accused. You may also seek police protection in the alternate. In the event you want the case transferred from one district to another, within one state, you may need to approach Hon'ble High court for the same, however in the event case is to be transferred from a district located in one state to district located in other state, you may need to approach Hon'ble Supreme court of India for the same. You can seek a transfer of the trial from the High Court in case you want to get a transfer within a state and from the Supreme Court if in a different State. Danger to life would be a valid ground to seek a transfer.
