The Court has discretion to exempt from attendance as witness any person who in the opinion of the Court, is from sickness or infirmity unable so to attend (Order XXVI Rule 1, C.P.C.). You can explain court your situation and request court to exempt your from future appearance as witness. Submitting medical certificate can be helpful in your situation.

As u have signed and received the court summons, you are obliged to appear in court. However, it is the discretion of judge who can close your evidence for non appearance or choose to issue B/W. Considering the situation you are in, you could send your representative to appear before the court and take time citing your medical grounds. Which will be accepted.
