It depends on the purpose of using that working history . In few legal cases , anything written on the Affidavit is admissible as an evidence, whereas in service related cases, P.F and E.S.I cases it is invalid until it is supported by attendance sheet and salary slip.

It depends upon the company status. If company is multinational company then it must in proper form and on company letter head.
But if it's small traders then statement written over stamp paper will be sufficient.
It also matter that where you want to utilize that job history/experience.
Ask from the present employer that job history on stamp paper will be OK for them or not.
